Help diagnosing clutch problem?

Look like your "slave cylinder" is bad. The slave cylinder is what allow the clutch to engage and disengage. I would replace the entire master cylinder, slave cylinder, throw out bearing, clutch kit (pressure plate & friction plate), have the fly wheel resurface, and a great time to replace that rear main seal to the engine.
